• Registration fees include participation in scientific sessions, meeting materials and coffee/tea. Lunch is not included.

* Verification letter on letterhead from your institute/hospital is required to qualify for Trainees and Students fee.

Payment

Payment must be made in Hong Kong Dollars (HK$) by the following methods:

• Cheque: payable to “International Conference Consultants Limited”
• Credit Card: (in Hong Kong dollars only)
  *The Meeting Secretariat will send you an online payment in a separate email.
• Registration confirmation will be emailed to the registered delegates after receipt of full payment.

Cancellation Policy

Cancellations must be made in writing to the Meeting Secretariat and the refund will be made after the Meeting. If cancellation is received on or before 30 September 2018, 50% of the registration fee will be refunded. Thereafter, there is no refund of registration fee.

Rules and Regulations (Registration):

1. Each registrant should complete a separate registration form. Photocopy of the registration form is acceptable.
2. Registration form without payment will NOT be processed. Please do NOT send cash.
3. International Conference Consultants Ltd (ICC LTD) will send a letter of confirmation by email upon receipt of your registration form and full payment. Kindly check the letter of confirmation. Any changes or alterations must be made in writing to ICC LTD.
4. Cancellations must be made in writing to ICC LTD and the refund will be made after the meeting. If cancellation is received on or before 30 September 2018, 50% of the registration fee will be refunded. Thereafter, there is no refund of registration fee.
5. The program is subject to change without prior notice. In the event of cancellation of the Meeting, the only liability of the Organizers is to refund all the fees paid.
6. All enquiries, changes and cancellations should be made in writing to ICC LTD.
7. All the payments are in Hong Kong Dollars only. Exchange rate is approximately US$1 = HK$7.75.

Limited hotel rooms at special room rates for participants attending the Meeting have been reserved at the following hotels which are ideally located in the neighbourhood of the Meeting Venue (HKCEC) and are available on a first-come, first-served basis.

Booking Notes:

1. The above rates are subject to 10% service charge.  Government tax is not required as of today.
2. To guarantee the special discount rates, reservations must be made through International Conference Consultants Limited (ICC). Credit card details are required by hotel to secure your booking.
3. Hotel reservations will be taken on a first-come, first-served basis and subject to room availability.  ICC has an obligation to return all unsold rooms to the hotels after 14 November 2018.Therefore room availability and special rates for reservations made after that date cannot be guaranteed.
4. The above hotel rates are only valid during the meeting period for delegates attending Ovarian Club XII.All bookings for days outside the meeting period will be subject to availability.
5. All enquiries, changes or cancellation of room reservations should be addressed to ICC and NOT directly to the hotel.
6. Cancellation Policy:

Cancellations before 14 November 2018	One-night room charge will be charged as cancellation fee.
Cancellations, change or shorten period of stay after 14 November 2018	Full room charge of entire period of stay and/or unused nights and/or early departure will be charged as cancellation fee.

Remarks:

• The travelling time between Hong Kong International Airport and the above hotels is 45-60 minutes by taxi, approximately 40 km.
• Early check-in and/or late check-out will be subject to availability and additional charge will apply.
• If you are crossing the International Date Line, please take into consideration the time difference and check that your arrival date in Hong Kong is correct.
